Throughout the year, many kinds of forms are sent home to be completed and returned to school. Important forms that will go home at the beginning of the year are : lunch applications, emergency dismissal, over-the-counter medicine approval, walker form, and an affidavit of residency with a utility bill attached as proof. Please return them to school in the red vinyl folder. Please keep the Student Handbook at home.
In addition to these school forms, I will be sending home a permission slip regarding displaying my students work on my website. Please be assured that your child's privacy will be honored. We are not allowed to put both pictures of children and their names together on our websites. You have the choice of what you want me to do.
As part of my continuing education as a professional, I may be taking pictures of my students, using their work, and videotaping lessons. These will be used to improve my teaching and will be viewed by other teachers. I will also be using their pictures in classroom projects or displays. A permission slip regarding this will also be sent home.
