Student Transfer Request

Use this form to request that a student be WITHDRAWN from their current school and ENTERED into a different school.

Please note the following important expectations that you will need to agree with to submit the form:

• I understand that NO TRANSPORTATION (BUS) will be provided. Entiendo que NO SE PROPORCIONARÁ TRANSPORTE (AUTOBUS).
  Tomei conhecimento que NENHUM TRANSPORTE (AUTOCARRO) será fornecido.
• I understand that the child/children must ATTEND school on a REGULAR BASIS, ON TIME each day. Entiendo que el/los niño/los niños deben ASISTIR a la escuela de manera REGULAR, A TIEMPO todos los días. Entendo que a(s) criança(s) deve(m) FREQUENTAR a escola REGULARMENTE, NO HORÁRIO todos os dias.
   
• I understand that the child/children must be successful ACADEMICALLY and BEHAVIORALLY. * Entiendo que el/los niño/los niños deben tener éxito ACADÉMICAMENTE y CONDUCTUALMENTE. Entendo que a(s) criança(s) deve(m) ter sucesso ACADEMICAMENTE e COMPORTAMENTALMENTE.

• I understand that if the child/children fail to meet these expectations, they will be returned to their home school, or another school, determined by Central Office leadership. Entiendo que si el niño/los niños no cumplen con estas expectativas, regresarán a su escuela de origen o a otra escuela, según lo determine el liderazgo de la Oficina Central. Compreendo que se a(s) criança(s) não corresponder(em) a estas expectativas, será(ão) devolvida(s) à escola de origem ou a outra escola, conforme determinado pela direção do Gabinete Central.
   
• I understand that I will receive a decision (APPROVE or DENY) about this transfer request as as possible, and that decision will be sent to the email address in Skyward, and I will need to REPLY to CONFIRM the transfer. The transfer will not be made until that confirmation is received. If that email address does not match what is on this form, the request will automatically be denied since we cannot confirm that it is coming from a guardian. Entiendo que recibiré una decisión (APROBAR o RECHAZAR) sobre esta solicitud de transferencia lo antes posible. Dicha decisión se enviará a la dirección de correo electrónico de Skyward. Debo RESPONDER para CONFIRMAR la transferencia. La transferencia no se realizará hasta recibir dicha confirmación.Si la dirección de correo electrónico no coincide con la que aparece en este formulario, la solicitud se rechazará automáticamente, ya que no podemos confirmar que provenga de un tutor. Compreendo que receberei uma decisão (APROVAR ou NEGAR) sobre este pedido de transferência o mais rapidamente possível, e essa decisão será enviada para o endereço de e-mail no Skyward, e terei de RESPONDER para CONFIRMAR a transferência. A transferência não será realizada até que essa confirmação seja recebida.
  Se o endereço de e-mail não corresponder ao que está neste formulário, o pedido será automaticamente negado, uma vez que não podemos confirmar se provém de um responsável.
